The surgery has been custom built to provide the most up-to-date facilities including disabled access and toilet facilities, comfortable reception area, fully computerised services and a large car park.
APPOINTMENTS AND VISITS
SURGERY OPENING HOURS
Monday to Thursday 8.00am – 7.30pm (The telephone lines are open from 8.30am – 6.30 pm)
Friday 8.00am - 6.30pm (The telephone lines are open from 8.30am – 6.30 pm)
Surgeries are conducted at varying times within the opening hours.
The normal consultation time is 10 minutes.
The surgery is closed for all statutory Bank Holidays and weekends.
APPOINTMENTS
Appointments are available on the day
We offer appointments which can be booked everyday between 8.00-9.30am by coming into the surgery and you will be given a timed appointment for the same day. Or you can telephone between 8.30-9.30am to book your same day appointment to see a doctor. Your call may be recorded for training and monitoring purposes. These appointments are for patients who feel unwell on the day and feel they need to be seen urgently. Please inform the Doctor if you require a chaperone. For urgent problems on the day, when appointments are full you may be offered a telephone consultation with the Doctor On Call who will call you back during the day to assess the situation. Please assist reception by giving a brief description of your symptoms and ensure you give us your correct telephone details so the Doctor can contact you. All nurse appointments can be made in advance so please book as soon as possible.
Pre-bookable Appointments
Routine appointments for all on-going and non-urgent problems may be made in advance, which will enable us to offer you an appointment time more suitable to your requirements. All the late evening clinics are pre-bookable and are designed for patients who work and who need to see a Doctor non-urgently. If you cannot keep an appointment, please inform us as soon as possible.
Online Appointments
The practice introduced a system for booking appointments online called EMIS Access with effect from 1 May 2010. We do not have all our appointments online so not to disadvantage patients who do not have internet access but do attempt to have a selection available. We are able to offer GP appointments and not Nurses. Before you can start booking online you need to be registered on the system, you will be able to book appointments for yourself once you are registered. The system is available for under 16s and parents can register their children at reception and they will be issued with their own password. Please collect a leaflet from reception which will give you all the information you need.
On-line appointments are released daily so availability to book one week or two weeks in advance should make regular review easier for patients. The surgery continually adjusts how many appointments are offered on-line to ensure a variety of options are available. In the future, it is hoped to release more on-line appointments in advance for regular reviews and therefore the “Book on the Day” appointments will be for emergencies only.
If you are unable to attend an appointment you have booked on line, please notify the surgery by telephone to cancel so that they may offer it to another patient.
Home Visits
Home visits are for seriously ill patients who are genuinely to unwell to travel to the Surgery. If you need a home visit, please telephone between 8.30am and 10.00am so the Doctors can plan their work for the day. Please remember many patients can be seen in the Surgery during the time it takes to do a single visit.
Nurses Appointments
All Nurses appointments can be pre-booked so please give as much notice as possible for routine health screening or follow up appointments to ensure you have the best opportunity to get the day and time you require.
Out of Hours Cover
If you need medical advice after 6.30 pm in the evening or at weekends or on Bank Holidays, please telephone the normal surgery telephone number 01454 616262 and you will be connected to our Out of Hours service provider Brisdoc. They will triage your call to determine whether you require a telephone consultation, need to attend an appointment at a choice of clinics or whether you need a home visit. Contact Details
Please remember to inform us if you change your address, telephone number, mobile number or any other contact details. It is important we know how to contact you for many reasons such as needing to change an appointment time, sending important letters, urgent test results etc. Please complete the change of address form or see the receptionist to notify us of ANY changes. Due to confidentiality issues and in order to reduce the risk of errors we insist on any amendments to be given in writing so please respect this policy.
Confidentiality
The surgery is subject to the legal requirements of the Data Protection Act and strict rules governing the confidentiality of patient information are observed at all times. It is often necessary for members of staff other than Doctors and Nurses to have access to records in the course of their duties. In addition, limited information may be required by third parties for screening programmes. For further information please refer to the leaflet held in reception entitle “How we handle you information”. Please be aware due to Data Protection Act we cannot give out results or any other information for family members over the age of 16. If a relation or friend is happy for you to receive their results a signed letter must be written by the patient giving authorisation to allow results to be given. This will only be valid for one year and will need to be reviewed.
Zero Tolerance
The practice supports the Government’s NHS zero tolerance campaign. We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. This will be followed up by letter. If they persist, we will exercise our right to take action to have them removed from our list of patients.
The surgery is protected by CCTV cameras.
REPEAT PRESCRIPTIONS
Prescription requests must be made in writing to eliminate error. Notice of 48 hours will be needed before prescriptions can be collected. Requests are not accepted over the telephone – this is for your safety so please respect this policy.
There are a number of different ways you can request a prescription:-
1. Arrangements can be made with your local Pharmacy to request prescriptions on your behalf and you collect them from the Pharmacy ready made up.
2. Local chemists Bradley Stoke Pharmacy (based at Bradley Stoke Surgery), Billings Pharmacy, Boots - Willow Brook Centre, Boots – The Mall, J Hoots at Patchway, Lloyds – Brook Way, Lloyds – Braydon Avenue, Morrison’s, Sainsbury’s and Tesco – Willow Brook Centre, will collect your prescription by arrangement.
3. Email us at prescriptionsbradleystoke@nhs.net. Please remember to give your full name, date of birth and address and clearly state the medications you require along with the strength and what form they take i.e tablet, injection.
4. In writing enclosing a SAE and we will send it to you.
5. By completing a prescription request form or the right hand side of your prescription and putting it in the post box in the entrance to the Surgery.
6. By faxing your request to 01454 619161
7. By using the online prescription request form.
There are certain types of medicine which cannot be given without consultation with either a nurse or a doctor. The doctor will need to see you at regular intervals regarding your repeat medication and your review date will appear on the right-hand side of your repeat prescription. Please make sure you make an appointment when this is due.
