The surgery has been custom built to provide the most up-to-date facilities including disabled access and toilet facilities, comfortable reception area, fully computerised services and a large car park.

APPOINTMENTS AND VISITS

Surgery Opening Hours

Monday to Thursday 8.30am to 1.00pm and 2.00pm to 8.00pm
Friday 8.30am to 1.00pm and 2.00pm to 6.30pm
Surgeries are conducted at varying times within the opening hours.  The normal consultation time is 10 minutes.

Please try to request home visits before 10.00am.

Please phone on the day for an appointment the same day between 8.40am and 9.30am or call in at the surgery between 8.30am and 9.30am. You will be given a timed appointment. There are also Doctors appointments which can be made in advance.

All Nurses appointments can be pre-booked so please give as much notice as possible for routine health screening or follow up appointments to ensure you have the best opportunity to get the day and time you require.

A number of Emergency appointments are available in the afternoons. If you believe you need to see a Doctor urgently please contact the Surgery from 2.00pm and you will be given a timed emergency appointment. Please ensure if you request one of these appointments it is for emergency cases only. If we have no appointments left a message will be put in for the Doctor On Call. You will be asked some questions by the receptionists as the Doctor will need some information before they contact you.
The surgery is closed for all statutory Bank Holidays and weekends.

The Practice provides a 24 hour service by using BRISDOC up to 10.30 and FRENDOC for the rest of the time. If you believe you need a Doctor out of Surgery hours, please telephone 01454 616262. All out of hours calls are triaged initially by NHS Direct. NHS Direct is available for 24 hour advice on 0845 4647.

REPEAT PRESCRIPTIONS

Please request repeat prescriptions in writing 48 hours before collection. Requests are not accepted over the telephone - this is for your safety so please respect this policy.

There are a number of different ways you can request a prescription:-

  1. Arrangements can be made with your local Pharmacy to request prescriptions on your behalf and you collect them from the Pharmacy ready made up.
  2. Local Chemists - Lloyds, Tesco Pharmacy, Sainsbury’s and Boots at The Mall will collect your prescription by arrangement.
  3. Email us at prescriptionsbradleystoke@nhs.net
  4. In writing enclosing a SAE and we will send it to you.
  5. By completing a prescription request form or the right hand side of your prescription and putting it in the post box in the entrance to the Surgery.
  6. By faxing your request to 01454 619161.
